SHORT-TERM CARE
G.E.M. Health Care provides short-term home health care services such as:
- Assisting patients in the transition from hospital or nursing facility to home, with a focus of improving health and independence
- Assist and educate in management of activities of daily living
- Assist with providing education on safety in the home environment
- Respite Care
LONG-TERM CARE
For the chronically ill or disabled and the elderly who might otherwise require nursing home placement, G.E.M. Health Care can assign Personal Care Aides and Home Health Aides, as determined by your physician to provide ongoing assistance with activities of daily living.
REFERRALS
G.E.M. Health Care can assist with the needs for:
- Physical, occupational and speech therapy
- Medical supplies, equipment assessment and safety evaluations
- Registered dietitians and/or nutritionists
